Types of FSSAI Registration

Personal information of the FSSAI Application Applicant -
- PAN Card: Copy of PAN is compulsory identity proof.
- Copy of Passport/ Driving License/ Voter identity card / Aadhaar Card (any of the Four)
- Passport size Photograph
Registered place of work from where the food business is being operated
- The latest Utility Bill(Water/Electricity Bill)
- Copy of Rent Agreement( if case of Rented property)
- Copy of property paper (if owned property)
Note - It’s may be a residential property, there is no problem in that but above documentation are required for the same.
Documents required for Basic FSSAI Registration:
- Form-A must be correctly and signed by all directors/ proprietor/ partners /Executive member of society
- Statement regarding food safety management system(FSMS) on the letterhead of the firm
- Evidence of Income determining the annual turnover of food business operator
- Nomination of the person selected by the company in Form No.IX and the copy of Board Resolution
- Self statement by Director
- Affidavit on Non-Judicial Stamp paper
In addition to the above following documents are also required for Central or State FSSAI Licence:
- Duly filled Form – B must be appropriately completed and signed by the directors/ proprietor / partners / Executive member of association
- List of Proprietor / Partners / Executive Members of association with complete address and contact details.
- Constitution of company AOA and MOA
- Analysis report as per the nature and business activity of the corporation
- Food and safety management
- Evidence of Income determining the annual turnover of food business operator
- No Objection Certificate from the local authority
- List of food category preferred to be manufactured
- Production unit Photograph
- Any other documents which can be a demand by authority on the basis of the requirement of the business..
What is FSSAI Registration Fee?
FSSAI Registration Fee is divided into the professional fee and government fee, Government fee officially charged by the government for the application fee and professional fee which is charged by the Lawyer/CA to prepare your request.

FSSAI Registration Form
FSSAI Registration or Food License has mainly 2 types of the forms i.e is Form A & Form B.

- Form A For basic FSSAI Registration - Form A is basic fssai registration form fill up by the food business operator. Its Comprises of an FBOs Business activities.
